Help Me Grow Yuba County can answer questions and assist with accessing valuable community resources that help your child’s growth and development. The Help Me Grow Care Coordinators (specially trained public health nurses) work with you to identify your child’s needs and refer you to local agencies that fit best for your family.  Services are tailored to each family and may include enrollment into a developmental screening program, coordination of community agency referrals, family navigation and more.
 

Our goal is to support medical providers in transforming the local healthcare delivery system and promote the development of a dyadic system of care to enhance the health and well-being of children and families in Yuba County.

Kindergarten Oral Health Asessment (KOHA):  California requires all kindergarteners to complete a Kindergarten Oral Health Assessment (KOHA) by May 31st of their kindergarten year. This simple screening helps identify dental issues early, when they're easiest to treat.

For providers: The KOHA is quick, easy, and an essential part of preparing children for school success. A healthy mouth means less pain, fewer missed school days, and better ability to learn and thrive.

Help every child start kindergarten with a healthy smile. We suggest setting triggers in your EHR's to educate and offer a KOHA to all children before they turn 5 years old.

Perinatal mental health affects the whole family. The Blue Shift Project of Yuba Sutter is here to help you support families through this critical time.

Resources & Referrals for Providers

We offer comprehensive resources including links to screening tools, clinical guidance, and direct referrals to perinatal mental health navigators. Our navigators provide families with additional support, connection to community resources, and help navigating services, complementing, not substituting for, medical care.

At First 5 Yuba, we believe in stopping problems before they start. Adverse Childhood Experiences (ACEs) and toxic stress can impact lifelong health, but these outcomes are preventable.

By focusing on upstream prevention, intervening early and building protective factors, we empower families, caregivers, and providers with trauma-informed strategies that promote healing. Through ACE screening, education, and supportive resources, we can all help children develop the resilience they need to thrive.

Protecting Children from Secondhand & Thirdhand Smoke

Secondhand and thirdhand smoke pose serious health risks to children, increasing their likelihood of respiratory infections, asthma, ear infections, and SIDS. The good news? You can help families create healthier environments without providing cessation services yourself.

Refer, Don't Just Hand Out Brochures

Studies show that when patients are actively referred to cessation support—rather than simply given a brochure—they are significantly more likely to access and succeed with quit services. It takes just minutes and makes a tremendous difference.

How it works:

  1. Ask about secondhand and thirdhand smoke exposure at every pediatric & adult appointment

  2. Refer patients directly to Kick It California

  3. Kick It CA does the rest—providing free coaching, resources, and support in multiple languages

You don't need to be a cessation expert. You just need to make the connection. Every referral is an opportunity to help a family, and protect a child's health.
